"Drink plenty of water" comes attached to every urinary tract infection, delivered by doctors, pharmacists, and the internet with equal confidence and equal vagueness. Plenty of water. More fluids. Stay hydrated. None of it tells you how much, on what schedule, or whether it changes anything once an infection has already started.
There is a real answer, from a randomized controlled trial, with a number attached. And the number is large enough to be worth knowing: in women who got recurrent UTIs and drank very little, adding 1.5 liters of water a day cut their infections roughly in half.
That finding comes with conditions that matter as much as the headline. Here is what the research actually shows, who it applies to, and where water stops being the answer.
The Trial That Put a Number On It
For decades, "drink more water" was plausible physiology without much evidence behind it. That changed in 2018, when a randomized controlled trial published in JAMA Internal Medicine tested it directly.
The design was narrow on purpose. Researchers enrolled 140 premenopausal women who had experienced at least three UTIs in the previous year and who were drinking less than 1.5 liters of fluid per day. Half were asked to add 1.5 liters of water daily, roughly six extra cups, on top of their usual intake. The other half carried on as before. Both groups were followed for twelve months.
UTI episodes: the water group averaged 1.7 infections over the year, compared with 3.2 in the control group. That is a 48% reduction.
Antibiotic courses: 1.9 in the water group versus 3.6 in the control group, close to half as much antimicrobial exposure.
Time between episodes: longer in the water group, meaning infections were not just fewer but further apart.
For a free intervention whose only side effect is more trips to the bathroom, an effect that size is unusual. But note carefully who was studied: women who were already drinking very little. That caveat is the most important thing on this page.
Why Your Baseline Matters More Than the Target
Here is the honest framing. The trial did not show that drinking a lot of water prevents UTIs. It showed that moving from too little to adequate prevents them.
Every participant started below 1.5 liters a day, which is genuinely low. The intervention roughly doubled that. If you already drink 2.5 liters, adding another 1.5 does not have the same headroom available, because the mechanism the trial exploited, moving urine through your bladder often enough, is already working for you.

So the practical question is not "how much water prevents UTIs" but "am I in the group this trial studied." If you routinely get through most of a workday without a bathroom trip, or your urine is consistently dark, the answer is probably yes, and the effect available to you is large.
The Mechanism: A Race Your Bladder Can Win
Most UTIs begin when E. coli from the bowel reaches the urethra and travels up into the bladder. What happens next is essentially a race.
The bacteria need to attach to the cells lining the bladder wall. Once they adhere and start colonizing, they are hard to dislodge and your immune system has a real fight on its hands. Before they attach, they are just floating in urine, and urine leaves.
Hydration changes the odds three ways.
Dilution: more fluid means fewer bacteria per milliliter of urine, so fewer are in contact with the bladder wall at any given moment.
Frequency: more urine volume means more frequent voiding, and every void physically flushes out unattached bacteria before they can establish.
Transit time: a bladder that empties every two to three hours gives bacteria a short window. A bladder that sits full for six hours gives them a warm, static, nutrient-rich environment and plenty of time to work with.
That third point is why the schedule of your drinking matters at least as much as the total.
Spacing Beats Total
Two people can both drink 2.5 liters a day and get completely different results.
The first spreads it across the day and empties their bladder every couple of hours. The second has a little in the morning, nothing through a long shift, then a liter in the evening. The daily total is identical. The number of flush cycles is not.
Distribute it evenly: a glass on waking, then roughly one every 90 minutes to two hours through the day, tapering in the evening if night waking is a problem for you.
Do not hold it: this is the single most modifiable habit for anyone whose job makes bathrooms inconvenient. Nurses, teachers, drivers, and people in back-to-back meetings routinely hold urine for hours, which is precisely the condition bacteria need.
Empty completely: rushing tends to leave residual urine behind, and residual urine is a reservoir. Take the extra few seconds.
Void after sex: intercourse mechanically moves bacteria toward the urethra. Urinating shortly afterward flushes them out before they travel further.
Wipe front to back: standard advice, still correct, still worth stating.

Prevention Is Not Treatment
This is the section that matters most, and it is exactly what the vague "drink lots of water" advice tends to blur.
Increased fluid intake reduces how often UTIs happen. It does not cure one that has already established. If you have burning, urgency, frequency, and cloudy or strong-smelling urine, extra water may dilute the discomfort slightly, but the bacterial population in your bladder is now reproducing faster than voiding removes it. Untreated bladder infections can ascend to the kidneys.
See a clinician promptly for suspected UTI symptoms, and treat these as urgent:
Fever or chills: suggests the infection has moved beyond the bladder.
Flank or mid-back pain: the classic sign of kidney involvement, called pyelonephritis.
Nausea or vomiting: same concern.
Visible blood in your urine.
Symptoms in a man: UTIs are much less common in men and generally warrant evaluation rather than self-management.

Drink water while you take the antibiotics. Do not drink water instead of them.
Where Cranberry and D-Mannose Actually Sit
Both get recommended constantly, and their evidence diverged sharply over the past few years. It is worth knowing which way each one went.
Cranberry has better evidence than its reputation suggests. A 2023 Cochrane review pooling more than 6,000 participants found cranberry products reduced UTI risk with a risk ratio of 0.70, and in women with recurrent UTIs specifically, 0.74. That is moderate-certainty evidence for roughly a quarter to a third fewer infections, and it reversed an earlier, more pessimistic Cochrane conclusion. The active compounds are proanthocyanidins that interfere with E. coli adhesion, the same step in the process that flushing targets. The catch is dose and form: standardized capsules and unsweetened juice carry the compounds, while sweetened cranberry juice cocktail mostly carries sugar.
D-mannose is the promising one that failed its test. A 2024 randomized, double-blind, placebo-controlled trial run across 99 UK primary care centers found no meaningful benefit: 51% of women taking d-mannose had another UTI, against 55.7% on placebo. The authors concluded it should not be recommended for preventing recurrent UTI in primary care. Smaller earlier studies had looked encouraging, and the larger, better-controlled trial did not confirm them.

When Water Is Not the Main Lever
The 2018 trial studied premenopausal women who drank very little. Several groups sit outside it.
After menopause: recurrent UTIs are often driven by declining estrogen, which shifts vaginal pH and the protective microbiome. Hydration still helps, but topical vaginal estrogen has the stronger evidence here, and it is worth a conversation with a doctor rather than another liter of water. With a catheter or a structural issue: an enlarged prostate, incomplete emptying, stones, or an indwelling catheter change the picture, and flushing alone does not address the underlying cause.
Already well hydrated: if your intake sits comfortably above 2 liters and your urine is consistently pale, more water is unlikely to be your missing lever.

Reading Your Own Feedback
You do not need to measure anything to know whether this is working. The Short Version
Adding 1.5 liters of water a day cut recurrent UTIs by 48% in a year-long randomized trial, but the women studied started below 1.5 liters total, so the gain came from fixing a real deficit rather than from drinking a lot. The mechanism is a race: bacteria have to attach to your bladder wall before they can colonize, and frequent voiding removes them first. That makes distribution across the day and not holding urine matter as much as the daily total.
Water prevents infections. It does not treat them, and fever or back pain means care today rather than another glass. Cranberry has moderate evidence behind it, d-mannose lost its best trial, and after menopause the strongest lever is hormonal rather than fluid.
